The Social Services Department of Laughlin Memorial Hospital is available to assist you in regard to any needs you may have when you are preparing to be discharged from the hospital. We will talk with you about any concerns you have in relation to continued care that you may need at home including self care, care by family members, home health care, sitters, or referral to other healthcare services including nursing home, rehabilitative care, assisted living, etc. We will work with your physician and you in providing the best possible plan for your discharge. We can assist you with the following:
Home health services
Obtaining home medical equipment/supplies
Nursing home placement- skilled/intermediate care
Assisted living care
Rehabilitative services- home care/inpatient
Support group information
Financial Assistance with your hospital bills
Information and referral to:
Community counseling services
Community social services agencies
Community resource information
Mental health services
We are also available to provide services to accident victims, drug, or alcoholic patients/families, the elderly, homeless, chronically ill, out of town patients/relatives, and abused children or adults.
