Social Services

Social Services coordinates admissions to the facility providing residents and family members with current federal and state information, resident rights, non-discriminatory policy, advance directives, etc and also does assessment of the needs of the resident seeking admission to the facility. Social Services works with individual residents and family members toward adjustment to nursing home life and to crisis, illness, disability and death. Also, Social Services serves as an advocate for residents and their families in expressing, defining and resolving grievances. Social Services coordinates discharge planning functions and when indicated, makes referral for home health visits for continued recovery, rehabilitation care upon discharge from facility, including making sure all other supplies/services needed at home are received. Each resident has different needs and social services advocates for each individually.
